The purpose of transition planning:

• To help make the transition from early intervention, at any time, as smooth as possible

• Timely IDEA Part C transition activities impact the timeliness of IDEA Part B services so they have enough time to complete their responsibilities

• To allow parents time to plan for transition, and learn more about available resources
