Service Coordination

Service coordinators (SC) meet regularly with the family to provide support for the family and ensure the following activities are  completed with the family:   coordinating intake, evaluation, eligibility determination, and development and implementation of the Individualized Family Service Plan (IFSP). This role facilitates and leads IFSP meetings, ensures services are delivered in natural environments, monitors timelines and compliance with state and federal regulations, maintains accurate documentation, supports families in identifying priorities and accessing community resources, explains procedural safeguards, collaborates with multidisciplinary providers, and coordinates transition planning as children exit the program. The position requires a bachelor’s degree in a related human services field, experience working with young children and families, and should have strong organizational and communication skills.
