Family Survey - Update from AzEIP

Hi ACT SCs,

I spoke with Alicia Sharma with AzEIP about the family surveys. She mentioned that SCs can look at alternative ways to support families with completing the family surveys, such as, emailing survey to the family and family emailing it back to the SC or AzEIP. The SC could also complete the survey with the family and email the survey to AzEIP.

Family surveys can be emailed to Annie Converse at KConverse@azdes.gov and Estrella Olivio at eolivio@azdes.gov.

AzEIP is working on a plan to have the family survey easily accessible for families.
