Good morning EIP Leaders - The Consent for Evaluation - Prior Written Notice form (GCI-1038B, GCI-1038B-S) has been updated, both English and Spanish.
Additionally, we have created a new form, Acknowledgement of Child and Family Rights (GCI-1115A, GCI-1115A-S) both English and Spanish which can be used in lieu of the last signature page of the Child and Family Rights in AzEIP Booklet.
