ASDB Eligibility
Eligibility Criteria for ASDB (Arizona State Schools for the Deaf and the Blind) Early Childhood Education Services
Vision
Children 0-3, who have a medically diagnosed bilateral, significant vision impairment, as determined by an ophthalmological examination, which may impact the child’s motor,cognitive, social, and/or language development will be determined eligible for services.
Hearing-
Children 0-3, who have a bilateral, sensorineural hearing loss, as determined by a pediatric audiologist, and/or a permanent conductive hearing loss which may limit their language, speech, and/or social development will be determined eligible for services.
Deaf Blindness
Children 0-3, with combined losses both in vision and hearing, as determined by a pediatric audiologist and ophthalmological examinations, even if both are mild, may qualify as deaf blind and be eligible for services.

Vision - Include eye report (not prescription for glasses), before referral, or attach eye report to referral online. Please fill out as much information as possible.

Hearing - Include audiology and ENT reports, attaching them to online referral. If reports are too large to attach, please fax to 520-770-3010.
